IRS regulations require that any vendor conducting business with the City of Fullerton must submit a completed W-9 form (Request for Taxpayer Identification Number and Certification).

Vendors must have a W-9 on file with the City before payment will be made.

Submittal Instructions

Submit a signed W-9 form to one of the following:
